The two clubs have agreed on a £65.5million deal for Diaz, who has two years left to run on his contract at Anfield.
Bayern will fork out an initial £61.1m, with £4.4m to be paid in add-ons, which are considered easily attainable by Liverpool.
Diaz will now depart the club's pre-season tour in Asia within the next 24 hours having been given permission to undergo his Bayern medical.
Liverpool have now raked in more than £190m in player sales (invluding add-ons) since Richard Hughes became their sporting director in June 2024.

Diaz had been the subject of interest from Bayern over the last few weeks, with Liverpool previously rejecting a £58.5m bid.
The Reds had been strong in their stance that the Colombian was not for sale, but they have reluctantly caved in with the player keen on a new adventure
On Saturday, Arne Slot confirmed that Diaz had been left out of the Liverpool squad against AC Milan due to transfer speculation.
The Reds were beaten 4-2 by the Italian giants in a pre-season friendly in Hong Kong, with Diaz not part of the team at all.

As the team was announced by the club, the omission of Diaz and Darwin Nunez stood out amid their recent transfer talk.
Post-match, manager Slot then revealed that Nunez missed out due to a minor fitness issue, not due to ongoing transfer talk.

But he also confirmed that Diaz did not play against Milan due to the continued speculation over his future.
Meanwhile, Nunez is expected to leave Liverpool this summer.
But Nunez is believed to prefer to stay in Europe.
The futures of both Liverpool forwards may well be linked with the possible arrival of Alexander Isak.

Their departures would help partly fund what could be another British record transfer from the club this summer, having already broken the record to sign Florian Wirtz for £116m.
Isak dealt Newcastle a massive transfer blow by telling the club he wants to explore his options away from St.
James' Park.
He was left behind for their pre-season tour in Singapore after what the club claimed to be a thigh injury'.
Newcastle boss Eddie Howe said there is 'no chance' of Isak joining them later on amid the growing speculation over a move to Liverpool.
A £150million price tag has been set by the Magpies, who have missed out on targets including Bryan Mbeumo, Joao Pedro and Hugo Ekitike.
Al Hilal are also interested in Liverpool's Nunez, while the Reds remain in talks with Bayern Munich over Diaz.
Selling Nunez and Diaz would help fund any official bid for Isak.

Contract talks between Isak and Newcastle broke down after he asked for a £300,000-a-week new deal as exclusively revealed by talkSPORT.
Along with Diaz and Nunez, Alexis Mac Allister was also not in the squad.
He is said to be continuing an individual programme, while new signing Hugo Ekitike had to wait for his debut.
Joe Gomez has returned home from Asia due to injury, with Wataru Endo absent too due to illness.
In the starting line-up, Florian Wirtz made his first start in public for the club after appearing behind closed doors.
