West Brom were knocked out of the Carabao Cup via a penalty shootout defeat on Tuesday.
Ryan Mason’s men followed up their league win over Blackburn with a painful 3-2 defeat to the Rams on spotkicks.
The most notable action of the first half arrived when Alex Williams missed a close-range header from Alex Mowatt’s corner on his senior debut for the Baggies.
West Brom scored the opening goal on 12 August when Isaac Price assisted Aune Heggebo with a well-crafted header.
The striker couldn’t miss and duly put Mason’s side 1-0 up in the 67th minute, but Derby equalised with the last kick of the game via Joe Ward’s free-kick.
West Brom didn’t have their shooting boots on from 12 yards, with Jed Wallace, Josh Maja, and Karlan Grant all missing their penalties.

Joe Wildsmith – 7
Derby gave the West Brom goalkeeper very little to do, but he deserves credit for maintaining his concentration throughout a relatively dull affair at The Hawthorns.
Joe Wildsmith couldn’t have done much to stop Ward’s impressive goal, before saving the Derby right-back’s spotkick.

The right-back defended well before providing a wonderful cross for Price, which led to Albion’s goal.
George Campbell – 6
West Brom summer signing George Campbell was defensively sound for large parts, but picked up a yellow card for a mistimed challenge in the early exchanges of the second half.

The 22-year-old defended successfully, and was admirably confident in possession for Mason’s men.
Alex Williams – 6
Williams looked confident on his senior West Brom debut, but missed a key opportunity to put the Black Country club in the lead when he missed a header off a corner from close range.
Ousmane Diakite – 6
The former Salzburg man protected his back four well, but Alex Mowatt’s creativity outshone that of the Mali international.

Like Diakite, Mowatt showcased his defensive abilities.
He confidently dispatched his penalty in the shootout.
Mikey Johnston – 6
The Ireland international enjoyed a lively second-half spell, producing two saves from Josh Vickers in quick succession, but he wasn’t at his clinical best.
Isaac Price – 8
The Baggies’ creator-in-chief produced an assist when his cushioned header ensured that Heggebo could not miss from close range.
Price produced yet another confident display, showing off his passing range.
Tom Fellows – 4
Tom Fellows has been eyed by Everton this summer and is used to being one of the stars of the show for West Brom, but on Tuesday, he was anything but that.
The fact that just one of the England Under-21 international’s seven attempted crosses was accurate, as per FotMob, epitomised a poor day at the office.
Aune Heggebo – 7
Heggebo failed to cause Derby many problems until Price provided him with an opportunity he simply couldn’t miss.

Jed Wallace (for Fellows, 69) – 4
The former Millwall man displayed his attacking intent almost immediately after replacing Fellows, but his shot was high and wide.
